Alternate Billing, Transfer, and Cancellation Forms

To transfer and existing membership, please use the Membership Transfer Form which must be notorized. Both transferor and transferee (new owner) must complete the form to transfer the account to the new owner. There will be a $50 charge to the transferee (new owner) for this change. The transferee must still submit proof of ownership (deed) as well as complete all new service documents located under New Service Request Link (application and easement). There may also be a $100 or $150 Membership Deposit added depending on existing membership deposit on file. 

To completly dissolve a membership, the member must complete the Membership Cancellation Form. **To resume the membership with DWSC, the new, or previous, member will be responsible for the cost of a new membership once the membership has been terminated. If you do not fill out this form you will still be liable for all water use and monthly fees. We will respond with proof of cancellation when you send in the form.
